The following is a list and brief description of proformas that are available as to download, to alter for your own needs, and print out for your use.
A full set of this paperwork is also available on a CD in the SSW Implementation Parcel.
Sent out by Intake Worker or admin person to client after intake call
Provides confirmation of SSW appointment and name of assigned worker
Explains SSW process and any local practical details
Sent out with Pre-SSW Client Questionnaire
Sent out prior to session, usually at intake
Completed by client and brought to first session
Encourages client to articulate goals for session
Informs worker what the client wants from the session and details specific
questions the client wants addressed
Filled in by worker after session to document the session
Used to summarise key details of the clinical session
Records questions worker wants to ask at follow-up phone call
Sent by intake worker or admin person to professional referrer - if client is
not self-referred
Informs referrer that your service has responded to the referral request
Explains SSW so that referrer does not think that SSW implies receiving
limited help
('script pad' format available from The Bouverie Centre)
Offered to client at the end of each session
Used to record what was useful from the session to inform the worker and
to act as a reminder for the client of things to try out
Can also be used to prescribe tasks, homework, strategies
Completed by worker during follow-up phone call
Provides a formal and consistent record of progress
Can be used to formally measure change from the Pre-SSW Client
Questionnaire
Filled out by worker during follow-up phone call along with the Post-SSW
Client Questionnaire
Used to summarise outcome and as a check list of actions pending
Accompanies the SSW Evaluation Form sent out by worker or admin person
Used to explain the purpose of the SSW Evaluation Form and invite participation
Client satisfaction survey sent out by the worker or admin person after the
follow-up phone call
Completed by client and returned to third party such as team leader for collation
Used to monitor and improve overall SSW service
Sent out by manager or person with SSW portfolio responsibilities once or
twice a year to staff involved in the SSW process
Used to inform management of the support needs of SSW workers and as
part of ongoing service development
